Online applications to our Book Cart Donation program will be available quarterly for one month.

The questions on the application are very easy and take less than 20 minutes to complete.

After completing Phase 1 you'll be invited with a link to complete Phase 2.

Deadlines for Phase 1:

• January 31st (Q1)
• April 30th (Q2)
• July 31st (Q3)
• October 31st (Q4)

This online form will outline a handful of questions related to anonymized data from your hospital.

Examples of questions:

• # of staff on your unit (total headcount)?
• # of fetal demises last year?
• How many of those were <20 weeks?
• How many of those were sets of multiples?

After completing Phase 2, you'll be given a few training videos to watch. These are 10-20 min videos on important perinatal bereavement topics. There will be a short quiz at the very end.

Depending on the size of your hospital unit, a certain number of staff will need to complete these trainings (~10%). Once you have enough personnel that have completed the training videos and quiz, your application will be completed & submitted for consideration.

• March 31st (Q1)
• June 30th (Q2)
• Sep 30th (Q3)
• Dec 31st (Q4)

During the 1st month of each quarterly cycle, our selection committee will meet to review the applications that have gone through all three phases by the appropriate deadlines. Gemma's Hope will donate $1,000 per quarter to eligible hospital(s). The amount given to hospitals will depend on their total size and needs. You'll be notified if you are selected or not.

Selected hospitals will receive their donated supplies shortly after being notified. Hospitals not selected will be notified & will be offered to remain in the applicant pool without any additional tasks.
